Registration Fees

$80 per child
$70 per child if 2 or more
25% discount for Military, Police, Fire and EMS
No practices or games will be played until fees are paid in full and a signed waiver is submitted.
No fees will be refunded after the first game.

Please Read the Following:

FLBC insurance is supplemental to the parent’s personal insurance. Parent’s insurance pays first, then the club insurance after that. Fish Lake Ball Club will NOT be responsible for medical bills not covered by insurance.
Parents are responsible for the transportation of their child/children to and from practices and games.
In order for the ball club to function properly, it is necessary for parents to help. It may be required that parents work in the concession stand at home games.

Liability Disclaimer Consent *

I, my heirs, executors and administrators, do hereby waive, release and forever discharge any and all rights and claims for damages which may have or which may hereafter accrue to me against the Fish Lake Ball Club, the sponsors, agents or representative, successors and/or assignees for any and all damages which may be sustained and suffered by me in connection with my said association with or entry and/or arising out of my travel to participate in and return from said Fish Lake Ball Club games conducted during the season.
